What is your appointment cancellation policy?

We request that you give at least a 24-hour notice if you need to cancel your scheduled appointment. A fee may be applied for any missed appointments.

Do you need to get a referral?

Please call your insurance company to determine whether you require a referral. If they are unable to provide that information, you may call our office and we will be happy to assist you.

How do I get a referral?

Your primary care physician’s office is responsible for issuing referrals. You will need to contact your primary care physician prior to making your appointment. They will provide you with the actual written referral and/or fax a copy to our office.

What do I need to bring for my first appointment?

Please bring the following:

  • Your insurance card
  • Photo identification
  • Referral
  • Completed and signed forms (patient history, registration, acknowledgement of privacy)
  • Bottles of all medications including prescription medications, over-the-counter medications, vitamins, and herbal supplements
  • Any old medical records, laboratory data, and results of previous testing that you have available.

What do I need to bring for my follow-up appointment?

Please bring:

  • Your insurance card
  • Updated address
  • Updated telephone number
  • Bottles or complete list of all medications including prescription medications, over-the-counter medications, vitamins, and herbal supplements
  • Any new medical records, laboratory data, and results of recent testing

What do I do if I need help after regular business hours?

If it is an emergency, call 911.

Can I get a refill of my medications?

It is of great importance that you do not run out of your medications. In most cases, if a prescription runs out, it does not mean that you no longer need to take the medication. Do not stop any of your medications without discussing it with your physician first. Check your prescription bottle or with your pharmacy routinely to keep track of how many refills you have left. Once you are aware that you do not have any more refills, call our office. We will provide you with a new prescription or we will call your pharmacy. Do not wait until you have run out of your medications before calling for refills.

How can I get my records copied or transferred?

If you need to have your medical records copied, you must sign a release of information form. Once a release has been signed, the request will be handled by our medical records department. Pertinent parts of your record will be made available to you or forwarded to another physician involved in your care.

What are your billing procedures?

For each appointment, we request that you be prepared to show your current insurance card, pay your co-pay, and pay any co-insurance or deductible. We accept cash, personal check, money order, or credit card payments. We encourage you to understand the details of your insurance coverage as each insurance company requires different co-pays and deductibles. As a service to you, we will file all primary and secondary claims for you to help you receive the maximum reimbursement under your policy. Billing statements are sent monthly. Statements are not generated until your insurance company has processed the claim submitted.

How can I make a suggestion, compliment, or complaint?

You may make a suggestion, compliment or complaint in writing, addressed to the Practice Manager. You may also speak directly with the Practice Manager either during your office visit or over the telephone. Be assured that any complaints will be kept confidential and will be treated seriously. We value all input from our patients and will use that information to serve you better.
